Status at Discharge:   These tables and figures show the distribution of categories of discharge status of individuals discharged from ACT. These reports compare the one year period through today with the one year period ending one year ago. In the figure, the first bar represents one year ago and the second bar represents today.
Teams categorize a discharge using a specified set of options which are aggregated into the following 6 categories of discharge status:
Treatment Objectives Met - Recipient made substantial gains in their recovery and goal attainment such that recipient substantially met their objectives;
Recipient Found Alternate Services - Recipient found alternative services;
Recipient Discharged to Long Term Health Care - Recipient needed long term health care such as psychiatric hospitalization for longer than 3 months, nursing home, medical hospitalization, assisted living, or rehabilitation facility;
Recipient Not Engaged - Recipient terminated services or recipient did not appear for services for an extended period of time;
Recipient No Longer Available For Services - Recipient jailed, recipient moved from service area or recipient died;
