ACT team members complete a standardized assessment for each recipient within 30 days of admission and every six months thereafter until discharge. The assessments capture recipient demographic characteristics; living situation; educational and vocational activity; engagement in services; incidence of significant events such as hospitalization, homelessness, arrest, and incarceration; functional impairment in the areas of self-care and social skills; and any incidence of harmful behaviors.
This information is entered into the Child and Adult Integrated Reporting System (CAIRS). CAIRS is a secure and confidential HIPAA compliant information system developed and utilized by the OMH to record, facilitate, monitor, and evaluate the process of managing and coordinating mental health services to children and high risk individuals in New York State.
